CURRENT SEVENTH GRADERS ATHLETIC SURVEY
Parents & Students in 7th Grade,
My name is Melissa Crosse and I am the Middle School Activities Director for Johnston! I am very excited to share the opportunities your student can participate in, starting next year, in 8th grade. You will receive emails from me throughout the year, regarding athletics, music and club opportunities.
Please complete the survey below if you are interested in participating in the sports listed below for Fall 2023. This information is used to collect data of number of students, jerseys/helmets needed, provide you with updates for all fall 8th sports, 8th summer opportunities and more!
All students will need a Sports Physical and a 2023 Registration to be eligible to participate. I highly suggest scheduling a sports physical this summer, to ensure your student is ready for the first day of practice in the fall. Registration will open July 1st – 5th, 2023, however jerseys and equipment will need to be ordered this May.
I will send additional details and information this summer. If your student is interested, please complete the form below. Please note, your student can change sports or drop at any time, this is for preliminary numbers. If your student is not interested, please disregard this email.
